Your Rehabilitation Care Team

The best way to help you meet your goals is through a team approach. With your input, we will help define your emotional and physical needs, assess your strengths, limitations and refine and reinforce your skills. This is important so that you can succeed when you leave Adventist HealthCare Physical Health & Rehabilitation. We treat you as a whole person with a life... friends... problems... joys...and a need to be independent again. Below is an introduction to the team members, so that you have a good idea of who we are and how we work with you. Some of the members listed may not be involved in your specific care. You may notice that a given skill or task is the focus of more than one kind of therapy. This is to make sure that we cover everything you need.

You

You are the most important member of the team. We depend on you to give us the background information we need to assist in the collaboration of your course of rehabilitation. We also depend on your enthusiasm. Your active participation will make your progress go more smoothly. Without your support, the rehabilitation process just does not work! Rehabilitation is a collaborative process! Above all, ask questions. Please make sure to tell us if you do not understand something. If you have a concern, let us know. We want to help, and by working together, we have a greater chance of resolving your concerns.

Family/Friends/Caregivers

You may be asked to identify support person(s) who will be assisting you after discharge. The case manager will be working with you and your support team. Your team is welcome to schedule a family conference with our team of rehab specialists. Please talk with your case manager if you are interested. Toward the end of your stay, your support team may be asked to attend therapy sessions. Before attending a therapy session, please refer to the Inpatient Therapy Gym Etiquette page. The case manager or therapist will schedule the training session with the primary caregiver(s). The purpose of their attendance is to help them learn how to safely assist you at home. Your support team is encouraged to take an active role in your rehabilitation, but they must be specifically trained to do so. It is not safe for them to attempt to assist you out of bed or a wheelchair or walk with you until they have participated in one or more training sessions.

Doctor

Our rehabilitation teams are led by doctors that are called physiatrists. They are trained in physical medicine and rehabilitation. This doctor works with you and the team to build a plan of action. He/she translates your personal goals and needs into the areas that the team will focus on during your rehabilitation. This doctor also works with referring and consulting doctors to address the full scope of your medical and rehabilitation needs. We have over 150 consulting physicians available to support the medical needs you may have while you are with us.

Case Manager

Each patient is assigned to a Case Manager. The case manager will communicate with you and the team regarding insurance coverage, continuing care options, home versus outpatient therapy, and recommendations for medical equipment.

Discharge planning begins on the day you arrive. The interdisciplinary rehab team will meet weekly to discuss your progress. Each member of your team contributes to the plan of care toward discharge. Our goal is to prepare you to go back to the community.

The Case Manager will notify you of your projected discharge date and discuss the discharge process with you and your support system following the first Interdisciplinary Team Conference. Your options for discharge depend greatly on your ability to participate in rehab, medical condition at the time of discharge, insurance benefits, and personal preferences. Your Case Manager will discuss the results with you within 24 hours of the meeting. If questions or concerns arise, the Case Manager may refer you to a Social Worker to discuss options and provide community resource information. We can arrange for you to meet with the interdisciplinary rehab team if you have concerns or questions you would like to address. You can also request a meeting with the rehab team as necessary to discuss any questions and/or concerns related to your stay and the discharge process.

A day or two prior to your discharge date, your Case Manager will provide you with a green folder labeled "Discharge Instructions." This folder will hold information regarding your post-discharge services, instructions, and follow-up care such as appointments with doctor(s), outpatient therapy, homecare, medication(s), and diet. Discharge time is between 10am and 12 noon. The rehab team will make every effort to get your care arranged in advance so you can leave during that time.

Social Worker

The role of the Social Worker is to intervene with patients by assessing, planning and advocating for their health needs. The Social Worker will provide counseling to assist you and/or your family in understanding and coping with the rehabilitation process. The Social Worker will meet with you to provide information about what you can expect during your stay and gather information to help you plan for your discharge, community resources, and psychosocial adjustment to life after discharge. They also facilitate patient orientation classes that give you an overview of what you should expect. The Social Workers also facilitate a variety of support and education groups for you and/or your family. Your Social Worker will recommend an appropriate group for you.

The Social Worker can assist you with completing forms for Advance Directives if you do not have one in place or if you wish to make changes to an existing one. If you want to appoint a health care representative or make a living will, we can explain these forms to you. If you have already completed these forms, we will need a copy within three days of admission. If you cannot easily access copies, we will ask you to complete another Advanced Directive, so that we know your preferences.  

Nurse

The Nursing Department is composed of registered nurses (RN), and certified nursing assistants. To determine the nursing care you need, your needs are fully assessed by a registered nurse on admission and throughout your stay. If you have a preference regarding any aspect of your care, please inform your nurse. We will attempt in every way possible, to meet your needs. Nursing is here 24-hours a day, 7-days a week. Each shift, a nurse and nursing assistant work as a team with you to help you achieve the best outcomes for your recovery. The nursing staff will educate you about patient safety, your medications, bowel and bladder management and pain management. Nursing, in collaboration with occupational therapy, will also help you with bathing and dressing activities. Please wait for assistance before getting up. We are concerned with your safety, so in the beginning we may help you with many of your activities. Please check with us and ask for help to move around. Most patients start their day around 5:30am-6:30am. Scheduling a shower can be done in collaboration with nursing or occupational therapy as part of your plan of care.

Therapy

Therapy sessions are scheduled between 7:00am and 4:30pm, seven days a week. Sessions last between 30 minutes and 90 minutes for each discipline. Your therapy schedule will be distributed to you at breakfast. Most patients have both morning and afternoon physical and occupational therapy sessions, and may also have speech and recreation therapy as needed. We will show you where therapy areas are located on your first day. The first day of therapy will involve evaluations from physical therapy and occupational therapy. Speech language pathology and/or therapeutic recreation will also do evaluations as needed. You are encouraged to take your pain medication before therapy to promote participation in the therapy session. You are also encouraged to bring yourself to therapy by wheelchair or walking if cleared by physical therapy. Long handled pieces of adaptive equipment used for bathing and dressing may be recommended for purchase by your Occupational Therapist (OT). Kits are usually recommended for those recovering from hip replacements or other orthopedic/neuralgic disorder. Please check with your OT for details and prices.

Physical Therapist (PT)

Physical Therapy, or PT, focuses on mobility to improve your level of function so you can achieve the highest level of independence. The physical therapist will develop a program with you to work on strength, endurance, agility, balance, and coordination. The areas in which the physical therapists will work with you include: walking, transfers, and bed mobility. Any of the above areas may be addressed in an individual or group session. Any special equipment needs will also be assessed. The physical therapist will identify equipment that may help you move more easily.

Occupational Therapist (OT)

Occupational Therapy, or OT, focuses on improving your independence, Activities of Daily Living or ADLs. The specific areas that the therapist may assist you with include: increasing your eating skills, grooming, hygiene, dressing, and bathing. Other areas that may be addressed include functional bathroom transfers and home management skills. Any of the above areas may be addressed in an individual or group therapy session. Any special equipment needs to assist with ADLs will be assessed by the occupational therapist. The main goal is for you to resume your meaningful activities or occupations you do daily.

Speech-Language Pathology (SLP)

Speech-Language Pathology, or SLP, focuses on evaluation and treatment in the areas of communication, swallowing, and cognition. Treatment programs are designed to address problem areas such as understanding others; communicating wants, needs and ideas; reading; writing; memory; judgment; and attention. Swallowing therapy is provided as needed to people who have impaired ability to safely swallow food and/or liquid.

Therapeutic Recreation (TR)

Recreational Therapy, or TR, focuses on one's previous leisure activities to increase physical, cognitive, emotional, social, and spiritual functioning. These areas are addressed through individual assessments, one on one treatments, group sessions, education and community reintegration programs. This therapist can also recommend equipment necessary to participate in recreational activities.

Neuropsychologist (Psychologist)

Sometimes adjustment issues can arise while you are going through your rehabilitation. A neuropsychologist may meet with you and/or your family to provide psychotherapy and/or psychoeducation if you need emotional or psychological assistance. If you have a stroke, brain injury, or other insult to your brain, the neuropsychologist may assist in behavior management, i.e., helping to provide you with the most beneficial environment and structure to allow you to participate at your best in rehabilitation. The neuropsychologist will also be a leader in providing crisis intervention if you need such help. We find that when you know what happened to your brain, your experiences become more understandable to you. The neuropsychologist will be available upon request to give you or your family information about your brain, the healing process, and ways to cope with the necessary changes you are now facing. At times, the neuropsychologist may ask you to be seen by a psychiatrist if it appears that medication may ease emotional distress. These services may include counseling for patients and/or families for  crisis intervention. In addition to the Neuropsychologist, a Social Worker may offer ongoing counseling services to you and your family.

Chaplain

Spiritual Care Mission:

  • We meet the religious, spiritual and emotional needs of our patients, families and the health care team.
  • We accept and work together with all denominations and religious affiliations.
  • We commit to protecting patient privacy and confidentiality.

Spiritual care is an integral part of the healing process, especially when there is an acute situation in your life and the life of your family. A chaplain is someone who "enters into" this difficult time to listen, offer support and comfort, and as requested, spiritual care.

The chaplain sees those of all faith traditions or no singular faith tradition.  Also, there is a Rabbi and a Roman Catholic Priest who make weekly rounds,  as well as numerous Eucharistic Ministers. He or she will assist you and your family to find your unique, spiritual pathway toward wholeness and healing. Other professionals provide services as needed, and work with the treatment team in delivering comprehensive care. These professionals include:

Team Conferences

Team conferences will be held weekly with your therapists, nurses, case manager and physician to discuss your progress. Your case manager will discuss the results of this meeting with you. Together, we can work to further your successful reentry into the community as much as possible.
