Community Partnership Fund Application

To be considered for an Adventist HealthCare Community Partnership Fund grant, your organization must submit a formal grant application in the designated format via the form below.

Step 1 - Prepare Grant Application

Prepare your grant application for submission in electronic format. File types accepted for submission include MS Word (.doc) and Adobe PDF (.pdf). Please ensure that your grant application includes the following information or it will be rejected:

  1. Organization name
  2. Contact information for the organization (e.g. Executive Director) and an alternate contact person
  3. Information related to your non-profit status (e.g. Tax ID#, EIN, etc.)
  4. An executive summary of the proposal (1 page max)
  5. Mission statement or brief overview of the organization’s primary objectives
  6. The primary and secondary geographical areas served by the proposal
  7. Proposal information (2-4 pages max), to include,
    • Project description
    • Needs assessment
    • Proposed program outcomes
    • Project sustainability
  8. Financial information
    • Line item budget for the project
    • List of other funding sources, in-kind donations or other support
    • Plans for future funding (if applicable)
    • The amount requested from Adventist HealthCare Healthy Community Fund
  9. Board of Directors names

Step 2 - Submit Grant Application

Once your application is completed, please submit Grant Application online. Upon submission, you will receive an onscreen notification acknowledging receipt of the submission of your application.
