How to Schedule an Appointment
When you need an Imaging appointment, please call the scheduling phone number and a team member will be happy to help you find the most convenient appointment available that meets your needs. Be sure to have your insurance card and physician order or referral available when calling to schedule. This will ensure your appointment is scheduled without delay.
What to Bring to Your Appointment
In order for your appointment to run smoothly, please bring the following items to your appointment. If you don’t have the necessary information, your appointment time could be delayed.
- Insurance cards
- Referral or order from your physician
- Any scans or reports from outside of Adventist HealthCare, if needed

Find Suite 203 in Medical Plaza 2 on the campus of Adventist HealthCare Shady Grove Medical Center on Medical Center Drive. You’ll have access to other specialized services and providers at the hospital, if needed. With validation, onsite parking is free for the first three hours of your stay.

Visitor Safety Policies
If you bring a friend or family member to the imaging center, they must stay in the waiting room during your appointment. Unless accompanied by another adult, please do not bring children under 10 to the imaging center.
