Your First Appointment
During your free assessment, you’ll meet with a registered nurse (RN). Your nurse will get to know you and your social, mental and physical needs. Then, you’ll work together to create a personalized care plan.
Every 45 to 90 days, an Adventist HealthCare Home Assistance RN will work with your CNA or CMT to make sure your care plan is best meeting your needs and make changes if necessary.
